2 page form

Ok so I know there will be a simple answer to this but my brain can't get
there on Friday. I created a form that needs to run on to 2 pages. How do I
get a header and footer to show up on 2 pages. Right now my form prints on 2
pages and I'm not getting the header/footer

A Form?
Forms are for entering and manipulating data.
Reports are for printing.

Create a report. Print that.

If you simply wrote Form above but meant Report, then which header and
which footer are you talking about?
The Report Header ONLY prints on the first page of the report.
The Report Footer ONLY prints on the last page of the report.
If you use a Page Header and/or a Page footer, then those do print on
every page, unless you have set it not to.

however, if you like to print from forms (reports are better to print from)
you have to go into the design view and establish the page header and
footer. then put what you want to show on each page in where you want it to
show (top or bottom) header or footer.
