That's a lot of info to fit on a small card. You'll have to use a small
font, which makes me glad I'm not going to have to read it. Even with
bifocals, it could prove challenging. I'd suggest rotating the card to take
advantage of the longer side.

I would use Word, select Tools > Letters & Mailings > Envelopes and Labels >
Options > Avery 5388 and copy and paste in your information. (the steps may
be different depending on the version you're using - in Word 2007 just use
the Mailings tab). The template has 3 cards on a sheet, 3x5 each, so copy it
in once, do all your formatting, then copy and paste to the other cards on
the template.
