Accepting credit card information on my website

How do I design a simple one page sheet that I can post on website so that I
can integrate it with my merchant account to receive credit card information
and submit for approval on my website?

Do you know where I can download and modify a template in Word if it is
already available?

You need to be Googling for a third party e-commerce solution. Word isn't
going to do this.
