I have a form that I have created on a database. It's a
medical form, and at the top I have the company logo
(bitmap image). When I print preview the form, it is
perfect, however on the next record, and the next, and the
next it is not there. How do I have the header stay on all
of the record within the database?

When printing forms (bad idea IMO) you need to put things in the page header if
you want them to repeat (not the same as the form header).
