Accessing the Database properties-Notes in an ERD

I'm fairly new to Visio so I'm not sure if this is possible or not.

I'm trying to write VBA code to walk through each entity on our ERD and
extract the value of the 'Definition / Physical Name' and 'Notes' field that
appears in the 'Database properties' section for each entity.

I can step through and show each shape and identify which shapes I would
need to look at, but can't see how these other bits of information are
exposed. Is this possible, and if so would it be possible to see a demo code
snippet?
