Add Check boxes in an Outlook email

I'd like to create a template that includes checkboxes for a questionaire
email that could be replyed back to me. My problem is that the recipient
doesn't see the checkboxes. Thanks, Michaelcip

Michael,
the problem that will occur is that some people have their e-mails setup as
HTML, rich text and others as plain text.. Plaintext will have a hard time
recognizing custom forms.. A good idea is to maybe attach your form as a
document?
