add icons to the address cards

I want to show icons next to the names of people, on the
heading of the address cards. Should I change the design
of the form? How can I make them visible in the cards
view?

There is no known method in Outlook for doing this.

I saw it done but could not figure out how. Can it be a

As I said, there is no mechanism in Outlook for this. The application you saw may have done something at the WIndows level.

Are you asking if you can add the contact's pictures to the Contact
Form, and then have these show in some kind of folder view. You can
use the picture control to add the picture to the contact form, but I
don't know of any folder view that will show them.
