Add or Update the Wrong Field

I have had random errors occur where data collected from
form controls are saved via a record set to a single
table. The table and form had several new parameters
added without difficult EXCEPT on random occasions where
the module will start adding or updating and be off by 1
field (next field to the right in the table). All files /
PCs were updated along with the file where the table
resides. All links were refreshed and all files were
compacted and repaired.
Any ideas???
