add picture to appointment form.

What is the best way to add an image to a form that will be specific to that
appointment? An example would be a copy of a faxed itinerary for a specific
meeting. Some things that I need to add are in .PDF .MDI & also HTML. So I
figured that it would be easier to use images instead. Any suggestions?
