Add records to table or query from listbox with multiselect

I want to select multiple records from a list box for editing in a columnar
form, populating most of the fields in the form from records identified in a
list box. The columnar form would reference a different table from the list
box.

Most of the controls in the form would be combo boxes.

This is for a prescription writing program. The list box references a
database of chronic medications with associated doses, routes, and
instructions. The form references prescriptions to be created for these
medications.
