Adding attachments and a receipent list to an email form

Hi,

I have just designed my first email form - so far so good but now I am
stuck. When I receive a survey form with a particular response, I
want to send an email to someone telling them to look at this survey
form. The email form tells them what question has the response that
triggered the need for them to see the survey form.

1. The survey form is an image (actually two .tiff files). Is there
a simple way to attach these files to the email form?

2. There are a limited number of people who will receive the email
form, but the email form will only be sent to one person each time.
Is it possible to easily create something like a drop down list in the
"To" box on the email form?
