Adding tasks from Project Web Access

I'm working with adding a task to a project from Project Web Access. I follow
the "add a task" process, selecting the project, sub task, etc that I want
and add the task description and work effort and click Save and the update
the task list. When the task is Updated (I happen to also be the project
manager on this so I update my own tasks) it is added to the last project,
but not under the subtask that I specified. Instead it is added to project at
end of the plan as a subtask of something else.

Here's an example:

Task 1
subtask 1
subtask 2
Should be subtask 3 (the one I added as subtask of task one should go
here)
Task 2
subtask 4
subtask 5
Subtask 6
Task 3
subtask 3 goes here instead of under task 1

This is the normal behavior of the system as only the Project Manager can
make the final decision, and the system does not create dependency links
based on team member input.

Question: In this case, I am the project manager for two projects. For one
project the tasks will be placed under the subtask selected while adding a
task. On another project, the tasks are placed at the end of the project plan
(as a task or as a subtask seemingly random) rather than under the selected
subtask. This seems inconsistent. Is there a project plan setting for this?

Not that I'm aware of, and I've never been able to pin down the exact reason
for the inconsistent behavior.
