Adding tasks in "My task" view

Hello!
I am using Project Server 2007 without the timesheet. We are reporting tasks
using My tasks with the "hours performed per period"
I have a problem when a person adds new tasks and informs hours worked on
it. When the PM opens the project, the task is in there but the actual work
no. But this doesn't happen all the times. It is mostly with a speficic
project that have 1000 lines. Perhaps this has anything to do?
Does anybody know what's going on wrong?

For this, there are two acceptances required by the PM, the first to accept
the Self Assignment (New Task) and after accepting this, the PM should see a
second submission for the Actual Work against the NEW TASK.

Hi Sean, thank you for your answer. I forgot detailing than in my original
post.
The PM saw both submissions and he accepted them. The problem was that he
didn't see all that in the project.
