Can I add the middle initial field in the address book?

As a column? No directly.

If you select View in the Menu bar select Columns you can see what can be
added.

You could use one of the custom fields and use it as middle initial. If you
click on the custom 1 field in a contact, for example, you can edit the
name. However, the edited name does not show up in the column header when
you add it. Simply drag it between First Name and Last Name columns, drag
the divider bar and you don't even see the Custom 1 header and voila, you
have your middle initial.

Hope this helps!
