The Eighteen Carat Kid

i am trying to do something clever in ms word 2000 and i wonder if
someone can help please:

i need to make a form/template type thingy that users can open and
choose from one of two addresses. ie, they should have some way of
choosing - e.g., a button, or dropdown or something corresponding to
"establishment a" or "establishment b" and the address will appear
automagically. the form will be available online so it has to be
something that will travel easily (ie, autotext won't work)... oh and
security on the network is such that we can't do my favourite option,
which is to set up a macro for each, attached to a dropdown or toolbar
button. oh yes, and it has to be very technically undemanding - the
users will not want to muck about.

i tried some other workarounds but they were all either too clunky or
dropped dead as soon as the document went from my computer to someone
else's. can anyone suggest anything please? I'm sure it's simple... but
how is it done???
