AddressLayout Problem in Word 2002

Now that I thought I had this all figure out I used the Insert Address button
to insert a name and address into a document and--of course, I got something
goofy! Instead of the Display Name (which shows up in my Contacts as First,
Middle, Last) I got Last, First--but not consistently. For example, it got my
address and many others just fine (but without the dashes I inserted here for
clarity):

--Joseph W. McGuire
--Kelley Jasons McGuire & Spinelli LLP
--Suite 1500
--Centre Square West
--1500 Market Street
--Philadelphia, PA 19102

But it got a few wrong. 2 examples:

--Karpinski Irena [should be Irena I. Karpinski]
--1330 New Hampshire, NW
--Washington, DC 20036

--Abbot Edward [should be Edward P. Abbot]
--Smith Abbot LLP
--3 New York Plaza
--New York, NY 10004

I checked each of these Contacts to insure that I had their names entered
properly (First, Middle, Last). The AutoText item is shown here:

BTW, the names here are all attorneys and law firms in the public domain so
I can't imagine I am compromising anybody's privacy. The rest of the
AddressLayout seems fine--except the name. Any idea why this is happening?
If it makes any difference I am using Office 2000 through Microsoft Terminal
Server. I have Office 2000 at home and I do not have this problem with Word
2000 as far as I can tell.
