AddressLayout problem

I have redefined my AddressLayout autotext entry in Word
to include the fields I want to pull from my OAB:

{<PR_DISPLAY_NAME>}
{<PR_TITLE>}
{<PR_COMPANY_NAME>}
{<PR_STREET_ADDRESS>}
{<PR_LOCALITY>}, {<PR_STATE_OR_PROVINCE>}
{<PR_POSTAL_CODE>}

But the brackets are not suppressing the empty fields
(the article I used was written for Word 2000), any
suggestions?

Also, when I use insert address in Word, my OAB entries
are displayed first name, last name, I have tried
everything and can't find a way to change it to last
name, first name.
