Alternative to Outlook forms?

Hi,
I am having two main issues with the Form tool in Microsoft Outlook
2003.
1. Occasionally the recipient cannot open forms. (Recipent must delete
all FRMcache.dat files for form to load properly)
2. People using PDAs cannot open forms from their PDA.

I appreciate the functionality (esp the editable fields) of these
forms and am seeking an alternative. Does anyone have a template or
any other alternative form that can be stored in Outlook? Any
suggestions would be much appreciated.

1) Outlook version and build number?
2) You might want to ask in a PDA group, since this functionality would
depend on the software running on the PDA.
