Applying 2NF

Hello,
I am looking to add a large number of checkboxes to my "Company" form (bound
to tblCompanies). The names of those checkboxes are controled by another
table (tblModalities). When those boxes are checked, i would like for the
info to be stored in a tblCompanyModalities with the companyID and the
modalityID. Does anyone have any reccomendations on how to do this?
