Approvals going to wrong project manager

I have some projects in which once a team member submits his task it goes to
the wrong project manager for approval.

For e.g
I have a projectA whose manager is ManagerA.
ResourceA is working of this project.
Once he updates his task it goes to some other project manger for approval
and not to ManagerA

It is quite random.
I have check the status manager and it is correctly set.

Any help is appreciated.
