Are their other Outlook Journal entry forms besides the standard?

I am looking for other forms besides the standard provided in Outlook. I
need to add the contact name.

In Outlook 2007, the Contacts box for linking items to contacts does not
appear by default. You will need to turn it on -- Tools | Options | Contact
Options | Show Contact Linking on All Forms.
