Are there any templates ( Access/Infopath) for churches?

Using either Infopath or Access---I need a template that is designed for
church membership that includes personal contact information and join dates,
etc.

There are several of these around, but you might want to buy this book:
"Building MS Acces Applications" by John L Viescas (Microsoft Press, 2005).

The CD that comes with the book contains a membership application, and the
text documents it so well that it is easy to change it to do whatever you
need it to. Effectively, you pay for the documentation, and get the database
for free.

You may have to modify it to exactly meet your requirements but basically
everything is there.
