Are there guidelines for outlook application user interface?

Is there a document which list the guidelines with which I should use when
designing
my outlook add in user interface?

No.
Where to put my custom folders? Should it be under personal folders?

The right question is, "Should I put my folders in the user's default information store? Or should I create a separate local Personal Folders file for them?" Don't forget that many users have no "Personal Folders" -- they use an Exchange mailbox.
Should I disable the toolbar or hide it when moving to a different folder?

Whatever makes the application the most clear and least confusing to the user.
To which menu should I add items? should it be the File->New or the Action?
In essence what is the conceptual difference between the File->New menu,and
the Action menu?

Action is context-sensitive. It contains actions specific to the folder and to the particular type of folder. New is global.
