Ask Fields

I'm trying to reacquaint myself with making forms with ask fields. I know
how to enter the ask fields, the bookmarks, the prompts and how to make the
reference fields in the body of the letter refer to the bookmarks. But what
I've forgotten is how to get the prompts to appear each time I open the form.
Is it the way I'm saving it or some other very important thing I've
forgotten?

I hope the fact that you're the only book I read lately hasn't become too
big a burden! Thank you for all of your past help!

I suggest that you make use of a userform (more than one ask field would
drive me crazy)
