Attachment form that can change the hyperlink name from another fi

I have 4 fields.

-Employee ID
-Browse
-Document Name
-Comment

Problem 1: I what a taggle button to open a folder. Then when the document
is doubled clicked, I want it to input the hyperlink into the "Browse" Field.
Problem 2: I would like the “Document Name†to be inputted as the hyperlink
display name.

I thing I confused some people. I just wanted the 2nd thread disregarded.
Will someone please answer this question?
