That's right - it's not possible. An attachment requires encoding. If
Entourage were to encode it, all the "complex HTML" in your message would be
undone - you'd end up back with the message in plain text or without much of
the formatting. What happens when you create the message in Word and then
"Send to Recipient (as HTML)" is that the document is sent to the Tasman
HTML engine (that used to be in Internet Explorer) which Entourage normally
uses only to read complex HTML and it takes care of the encoding.
Entourage's own HTML engine and editor are "locked out". If you then let
them back in to encode an attachment, they'd undo everything that Tasman
accomplished.
This is obviously a workaround for Entourage not having its own full-blooded
HTML editor. I have no idea if there is something they could do to allow
specifying an attachment in Word, before sending the message to Entourage.
Try making a feature request via Help/Send Feedback. I'm afraid it may take
until Entourage has its own HTML editor - which might be a while.

Subject: Re: Attachments in "Send via HTML" from Word 2004 in Entourage
2004?
You¹re absolutely correct. I¹d never tried it before (I always send plain
text) but I have now, and I can¹t attach anything either. In fact, if you
expand the Attachments pane, it says underneath, ³The attachment settings
cannot be changed for this message.² I don¹t know why this is, but if Paul
Berkowitz doesn¹t jump in and explain it here, ask over on the
microsoft.public.mac.office.entourage newsgroup. It must have something to
do with the way Entourage creates this kind of message.
Sorry not to be of more help.
