Audit Trail

I am currently using an audit trail, but it appears that it it only updating
at initial data entry, i a new record. It is not updating any subsequent
changes made to the record in the following weeks.....
here is a copy of the audit trail (i did try allen brownes but the
installation of was beyond my ability, so have reverted to the exisitng
one....)
