Auto Populate Word Document

I need help!
Here is the problem. I have a Word document which I want to fillin patient
data.
I would like to type in the patient ID number and have the Word document
fillin the patient's last name, first name and date of birth.
The data for the patient information is stored in an excel spreadsheet with
the columns Patient ID, Last Name, First Name, DOB.
Can someone please direct me to a tutorial of give me the answer?

Create the document as a template and to the template add a userform on
which you have a combobox.
