Auto-Prompts for Form

I have a fax cover sheet that I created using Word 2003. I want to set up
prompts that would prompt the user with a dialog bog and then he/she would
fill in the appropriate information and then it would populate the
corresponding portion of the form. Does anyone know where I can find a
tutorial for this task?

Thank you for your well-done article below. I came across it looking for
solutions. I downloaded your sample file: InterviewLetter.dot. However,
when I open the file as a template or open it as a new document, the custom
dialogue box does not display. I figure I missing a step. Can you help?
