Autocorrect doesn't consistently work

A user of mine uses Word as email editor in Outlook 2003. Sometimes the
autocorrect works to correct words as he types while other times, it doesn't
autocorrect, even though it does underline it as misspelled. What could cause
this variance if the settings have NOT been adjusted in between uses?

Are they the same words? AutoCorrect isn't necessarily for
automatically correcting spelling errors. Granted most of the default
AutoCorrect entries are for common misspellings of words. Check
Tools/Options/AutoCorrect Options and see if the characters that were
typed are in the list. If they aren't then a correction will not be
made.

The specific example that my user complains about is changing teh to the
automatically. teh is in the list.

Since that is a standard entry I'm not sure. I'd need more details.
For example when it's not corrected is he still using Word as his
email editor? It's possible that is switching on him.

He does not change the settings since he is very dependent on spell checker.
How could it switch on him?
