Autoformatting a Form

I have created a form in word that will be used by multiple users each using
multiple forms. The users will be evaluating a document and will need to use
a new form for each paragraph they would like to make changes to.

Is there anyway for me to set up and auto fill with the person's last name
and current form number each time they open a new form?
