Bangtail Envelope Templates

I am looking for an easy template (or other means) to format a direct
mail/giving envelope for our not-for-profit agency. The envelope that we
want to use is called a 'Bangtail" meaning that it has a perforated flap just
under the flap that seals. Any ideas on how to get some help? Thanks!

You might want to check with a local printer that can do this work, to make
these envelopes, as a contribution to your organization.
