Best template for.....

I'm creating a database for a cemetery survey I did for a genealogy website.
I was wondering if somebody could recommend the best template for that
project? Thanks in advance.

I really doubt you will find a template for this. Have you looked at some of
the surveys already published online?

I've been using this site a lot and since I know for a fact that there are
lists of burials on this site, I'd suggest using the same general format.
