How can I keep a bookmark attached to text?

I inserted a bookmark and create a hyperlink to this bookmark at the
beginning of the document. But when if I cut and paste or insert some
carriage returns at the beginning of the bookmarked text, the text moves,
but not the bookmark. How can I keep these toegether?

Also, how can I tell what a bookmark is named? If I turn on the bookmarks so
I can see them in my document, there are some bookmarks that I don't know
where they came from. Is there any way to see this information?
