Build custom aspx association form (application page ) for WSS 3.0

Hi -
I am using a custom workflow developed in Visual Studio 2005 with WSS 3.0.
Currently I do not have an association URL defined in workflow.xml but the
server is taking users to the default Three-State Workflow association form,
which doesn't fit my workflow's needs at all.

Does anyone know where I could find an example of a simple aspx association
form, a tutorial or anything? I am coming up dry everywhere I look.
