I have just had this same problem. I was able to recreate it randomly. I am
currently calling my clients and I add a business note. If I get the voice
mail, I add a time stamp to the note and save it but don't close the note. I
then close the account though.
I then go to the next account doing much the same. I did 2 accounts this
way and the 3rd account when I opened it the Primary and Full Name columns
were gone.
I have tried this on both the Vista and XP platforms and on the 3rd or 4th
account that I open, the Primary and Full Name columns disappear.
I am currently using Outlook Business Contacts 2007 version 3.00.7311.00

somehow the fields displayed in this grid changed. I no longer see the name
of the contact, just the job title, phone and email.
How do I get it back and how can/could I customize this region?
