Button to add comments in a form

I am new to InfoPath and I am designing a form in which I would like to have
a button on the form when clicked would enable the user to type a comment and
then place that comment in a field that leaves his\her name with a timestamp.

Example:
I click a button and type my comment 'Joe, I need more information' it then
leaves that comment in a field or section that can gather all comments sort
of like an audit history. ...
Date: xx/xx/xx by:NAME1 comment: Joe, I need more information.
Date: yy/yy/yy by:NAME2 comment: next comment goes here
Date: zz/zz/zz by:NAME3 comment: another comment here
