Where can I get Calendar icon AND how do I apply it to Calendar Control in
Access form?

Add MSCAL.OCX into the References of your database and in the Design View of
your Form, select ActiveX Control ... and the Calendar will be on the list
you can select from.

BTW, this question should be posted in the "forms" newsgroup, not
"tabledesign".
