Calendar template location

Before I had a recent memory upgrade I had located a
calendar template that allowed me to do a year-at-a-glance
(all months on one page)rolling calendar (eg: start with
August '03 end with September '04). It was titled "Yearly
Planner"

I am unable to find the source for that template now.

Ideas???
