can i change default destination fields for field mapping

I created a new contact form but when I import to new form I can't change
default destination fields. Is there a way to add my newly created fields to
the mapping of imports

The Outlook (any version) import wizard does not support importing to
user-defined fields with or without a custom form in use - you'll need to
either write your own code or use a 3rd party product to do that.
