Outlook 2000
Users want the form on the first and only page of email. I've attempted to
do so but it is not working. Any ideas?

What in particular isn't working? Did you click the Edit Read Page button to create a layout for received messages? Where did you publish the form?

Hi Sue, quick KUDOS to you for your support!

I want to have the form as the only page displaying. Right now it is on the
second page (tab).

You can toggle the visible state of any page with the Form | Display This Page command.

I'm not sure I'm explaining myself correctly. I have created the form on just
one page (it was on a second page and the user had to click the tab). When I
send the form it does not display when the person opens their email. When it
is printed all the info is there but I need to be able to see it when it is
sent.
thanks!

Do you have separate compose and read layouts for that page? If so, dId you click the Edit Read Layout button and complete that layout?

Ok, I've answered my own question. What I did to fix the issue was to take
the form and copy it from the compose page to the read page. Now when the
email is sent and opened, the form is there!
