Can I set up questionaire and get responses in Outlook?

I'd like to set up a questionaire and get responses by e-mail. Everyone
involved with the questionaire has outlook 2003

There are Outlook functions for this if you are on Exchange Server. Ask
about that in an Outlook group.

You can use what Word calls an "online form." Check this in help. For more
