Can I use a form template document for mail merge?

I have created a form template document that has both checkbox and text form
fields. When I merge this document the check boxes come across, but the text
fields disappear. Does anyone know if this is normal and if it is, is there
a fix so I can get my text fields back?

You can unlock the document and lock the form fields before the merge. If
you need to relock the document as a form you would need to unlock the form
fields and relock using a macro (before Word 2003). Note that Ref fields and
calculation fields using your form field results may not work because the
bookmarks can only refer to one set of form fields.
