Can somebody help me with this?

I want to be able to setup a sign-in page before someone enters my website..

First Name:
Last Name:
Street Address:
City, State, Zip
Phone:
Cell:
Email Address:

Then I want the infomation send to me in a email, as the person enters my
website...

Is there a add-on to do this?
I have tried using forms in FP 2003, but I can't seem to get it to work....

You want a form with form mail enabled. It's all in the help files. You will
need formsmail enabled on your web server.
