Can you fill a template with information from contacts data base

I would like to know if it is posible to create a template that can be filled
with information from the contact data base. For example a Fax transmittal
sheet that only need to enter the person name and the rest of the information
is filled automaticaly

What application is your contact database in? Depending on the answer, you
may be able to use Word's mail merge feature. See:
