Canadian Rebate Request returned ... "Address unknown"

I completed all the documentation for a $150 CDN rebate for MS OneNote and
sent it to the Canadian address:

Microsoft Office OneNote 2003
Promotion Number 497-03-089
P.O Box 641
Fort Erie, ON L2A 6L8

It was returned to me today with a yellow post office label, saying "Moved /
Unknown. Return to Sender."

Since this offer is supposed to be good until September 2006, I do intend to
collect. Can you tell me the new address for this?

This should be the correct address. Would you mind providing your email

Just munge the address like sheila leave this out at whatever dot com. You
don't need to actually type in your email address.
