Cannot create team site for projects

I have experienced multiple problems with the team site creation for projects:
1) Based on the Site provisioning settings i have set - "Automatically
create a team Web site for the project when a project is published to Project
Server. ", a new site for the project should be automatically be created on
sharepoint server.
However, this does not happen

2) If the site does not exist, I should be able to go to Manage SharePoint
sites and "Create Site" for a project manually.
Even this does not work. I get an error:
"Error when creating site. Site does not seem to exist"
Here are my configurations:
In the "Connect to SharePoint server" link, I have:

SharePoint Central Administration URL: http://<servername>:3456 Test URL
Create a site under this SharePoint URL: http://<servername>:80/sites
http://<servername>:80/sites/MS_ProjectServer_PublicDocuments

In the "Site provisioning settings" link, I have the site owner email
addressed saved.
Based on the information in the KB article 555120

However, none of this seems to have worked.. The team site never gets created.
