Cannot find switchboard form

Hi
When I open the startup dialog and got the the Display
Form/Page drop down list the switchboard form does not
appear as a selection. In fact it is not visible in the
Forms either. Can you please help me find it?

If it exist, then you most likely have it hidden.

Go to Tools/Options/View and click the "hidden objects" checkbox.
