Cannot Forward message with Custom Form

Using an Organizational Form, and for some reason, my Outlook clients cannot
forward messages that open using the Form.

I believe I've seen this before, and there was some security setting
preventing Outlook from Forwarding Orgnizational Forms. Of course, I can't
find that setting anywhere.

Clients are running Outlook 2003 SP1.

Any help is appreciated.

Should also note that the error received when Forward us clicked is:

'Could not open one or more attachments.'

The message has a Wav file attached, and the Form is used to embed playback
controls for the wav file in the message so a wav player is not required.
