cannot make a template

I have a form for proposals I use that I orderred from
NEBS. I have used it for a number of years however have
always hand written them. I tried to make a template in
Word that I could call up and fill out-w-the information
for the prospective customer and after 3 hrd of trial and
error cannot figure it out. i would like just a skeleton-w-
areas that I can fill in and print out.
